About Stellar Health:

Historically, US Healthcare has relied on a fee-for-service reimbursement system where providers are paid based on the quantity of patient visits and procedures, rather than the quality of health outcomes.

At Stellar Health, we help primary care providers put patient health first. Our platform - a mix of technology, people, and analytics - supports providers at the point of care, delivering real-time patient information, activating practice staff, and empowering providers and care teams with incentives that reward the work they are already doing to keep patients healthy. Using the Stellar App, our web-based, point-of-care tool; practices receive a simple checklist of recommended actions that support the best quality care. Providers and care teams are then paid monthly for each action they complete, and Payors save money in reduced healthcare costs along the way.

What We Do

Our mission is making healthcare fair for clinicians, their teams, and patients in every community. Our current healthcare payment system has led to continuously increasing costs for inconsistent and subpar outcomes. To change this, we work directly with primary care providers and practice staff to deliver a step-change difference in value-based care through real-time action prompts tied to granular incentives. By directly incentivizing the people that directly care for the patient we unlock true alignment of actions and work that lead to these outcomes.

Our vision is that Stellar will be the essential, trusted platform for provider teams, making it easy and rewarding to sustainably care for all 300+ million Americans.** Stellar helps clinicians and staff understand what the best course of action is and why, and properly compensates the whole care team for the time it takes to do this work. Not only does our solution seamlessly integrate in care team workflows, it also aggregates the fragmented patient panels, care options, and care pathways - thereby making Stellar the essential and trusted platform for provider teams to sustainably care for all 300+ million Americans.

Why Work With Us

Working at Stellar means you get to be part of a significant, positive shift in how healthcare is delivered across the US. Your work will directly impact whether someone’s underlying health condition is managed effectively, while also helping doctors manage their chronic patients effectively.
