About Akido

Akido is a care delivery organization focused on providing personalized, holistic care to all, including vulnerable communities. By leveraging data and artificial intelligence, Akido is transforming the healthcare experience for its network of physicians, care providers, and nearly half a million patients across the nation.

Healthcare for the Gig Economy Workforce

Akido, the Independent Drivers Guild (IDG), and the Workers Benefit Fund (WBF) are coming together to launch an innovative healthcare initiative tailored for the underserved gig economy workforce, who often face barriers to accessing traditional care. The program is designed to meet patients where they are and deliver comprehensive “whole person care” that addresses physical, behavioral, and social drivers of health.

This inaugural program will launch in New York City this year, focusing on independent drivers, including those affiliated with platforms like Uber and Lyft.

About the Role

We are seeking a dedicated Case Manager who is passionate about patient advocacy and healthcare access. In this role, you will be the primary point of contact for gig workers navigating the healthcare system, ensuring they receive the resources, services, and follow-up care they need. This position requires strong problem-solving skills, adaptability, and the ability to work independently in a fast-paced, evolving healthcare setting. The Case Manager will collaborate with medical providers and community organizations to deliver holistic, patient-centered care while also forging long-lasting relationships with community partners.

You will be a trusted ally to patients, helping them navigate healthcare systems, addressing barriers to care, and ensuring they feel heard and supported. Your role will blend clinical care, patient education, and community outreach to build relationships and foster trust with those who need it most. If you are a problem-solver, enjoy hands-on patient interaction, and want to play a direct role in shaping a more just and accessible healthcare system, this is the opportunity for you.

What You’ll Do

Patient Advocacy & Care Coordination

  • Serve as a trusted healthcare navigator for gig economy workers, assisting with appointments, referrals, and service access
  • Conduct patient intake, needs assessments, and care planning to identify social determinants of health impacting access to care.
  • Assist patients in overcoming barriers to healthcare including insurance and financial concerns
  • Educate patients about preventative care, chronic disease management, and available community resources
  • Ensure follow-ups and continuity of care through scheduling assistance, referrals, and outreach effort.

Resource & Social Support Navigation

  • Connect patients to healthcare providers, mental health services, financial assistance programs, and other community resources
  • Work closely with team to address housing, food insecurity, and employment-related health concerns
  • Advocate for patients in accessing insurance coverage, benefits, and financial aid programs

Mobile & Community-Based Healthcare

  • Participate in mobile health initiatives, traveling across New York City’s five boroughs to meet patients where they are
  • Support community outreach events to raise awareness about available healthcare services
  • Work in both clinical and community settings, ensuring patients feel heard and supported

Collaboration & Innovation

  • Utilize AI, electronic health records (EHR) and digital tools to track patient progress and optimize care delivery
  • Provide feedback to improve program workflows and patient engagement strategies

These responsibilities are intended to describe the general nature and level of work being performed by personnel assigned to this job classification. They are not to be construed as an exhaustive list of job duties performed by personnel in this classification. Other job-related duties may be assigned by management.

Required Qualifications

  • Master’s degree in social work, public health, nursing, or a related field
  • 3+ years of case management, social work, or care coordination experience
  • Strong understanding of healthcare navigation, insurance systems, and community resources
  • Proficiency in EHR systems and comfort using telehealth platforms
  • Excellent communication, problem-solving, and interpersonal skills
  • Ability to travel across New York City and work in dynamic community-based settings
  • Fluent in conversational and medical Spanish

Preferred Qualifications

  • Master’s in Public Health (MPH), or a related field
  • Experience working with gig workers, immigrants, or other underserved populations
  • Familiarity with NYC-based public assistance programs and healthcare services
  • Experience with insurance navigation and Medicaid/Medicare enrollment

Work Environment

This role will require travel throughout all five boroughs of NYC bringing care directly to underserved communities. You’ll work in a variety of settings, including both indoor and outdoor environments, depending on the clinic’s location for the day. Flexibility is key, as this position may require nontraditional working hours, including early mornings, evenings, or weekends (final schedule to be determined). This is an exciting opportunity for someone who thrives in dynamic, on-the-go environments and is eager to be part of a healthcare model that meets patients where they are.
